Terminal illness is not a topic most people talk about. Yet, if this ever happens to you, it will be talked about by your family. Illinois law provides a way for patients to inform their Doctors to withhold medical treatment that would only serve to delay the inevitable. Otherwise, your family could keep medical treatment going for as long as funds were available.

This App is a one-page Illinois Living Will to declare your desire that if suffering from a terminal condition you prefer death delaying medical procedures be withheld.

It directs an attending physician to allow you to die naturally if your death is imminent. Otherwise, family members make the decision whether to prolong treatment regardless of your intent.

The full Illinois statute titled "Living Will Act" is included.

Illinois law does not require the document to be notarized, but this App includes provisions for a notary to prove your signature and that two witnesses were genuine.

Enter your name, address, date of birth, and social security number. The document is formatted as a PDF file for transfer to your computer via iTunes to save and print.
